Reisikõhulahtisus: mis seda ennetab
Profülaktika toimib halvemini, kui tooted väidavad, ja teadmine, mida teha, kui see juhtub, on olulisem kui selle ennetamine.
Mida profülaktika saavutab
Probiootilise profülaktika metaanalüüsid [1][3][4] ja probiootikume rifaksimiiniga võrdlevad analüüsid [2] leiavad haigestumuse tagasihoidlikku vähenemist. Tagasihoidlik on siin võtmesõna: need on suhtelise riski vähenemised sagedase seisundi puhul, mitte kaitse.
Antibiootilist profülaktikat ei soovitata üldjuhul tavareisijatele. See suurendab resistentsust, toob kaasa kõrvaltoimeid ja soodustab koloniseerumist resistentsete mikroorganismidega, mida reisijad seejärel koju kaasa toovad — populatsiooni tasandi kulu, mida makstakse individuaalse mugavuse eest.
Profülaktika reisijate kõhulahtisuse vastu
Toidu- ja veeohutuse abinõud
"Keeda see, küpseta see, koori see või jäta see" on kergesti meelde jääv, kuid vaid osaliselt tõhus — reeglite järgimist uurivad tööd näitavad, et ka reisijad, kes järgivad neid hoolikalt, haigestuvad märkimisväärsel sagedusel, peamiselt sellepärast, et kokkupuude toimub köökides, mida nad ei näe.
Kõige suurema kasuga meetmed: pudelivesi või töödeldud vesi, sealhulgas hammaste pesemiseks, teadmata päritolu jää vältimine, kuum toit, mis on serveeritud kuumalt (mitte hoitud soojas), ja iseenda kooritud puuvili. Käte pesemine enne söömist annab rohkem kui enamik toiduvältimise reegleid.
Kui see ei ole ainult reisijate kõhulahtisus
- Verine väljaheide või kõrge palavik. Pöörduge arsti poole, mitte ravige ennast.
- Sümptomid, mis kestavad üle nädala. Kaaluge parasiitseid põhjuseid, mis vajavad erinevat ravi.
- Raske vedelikupuudus, eriti lastel ja vanemaealistel reisijatel.
- Sümptomid, mis algavad pärast tagasitulekut. Öelge arstile, kus olete käinud; see muudab diferentsiaaldiagnostikat.
- Püsivad sümptomid pärast ägedat episoodi. Postinfektsioosne IBS on tunnustatud, kuid aladiagnoositud tagajärg.
Viimane punkt vajab tähelepanu: märkimisväärne vähemus arendab pärast infektsioosset episoodi püsivaid soolesümptomeid. Kui teie sooletegevus ei taastunud pärast reisi kunagi normaalseks, on see tunnustatud nähtus, mitte midagi, mille olete välja mõelnud — vaata meie soole-aju tõenduspõhist lehte.
Korduma kippuvad küsimused
Kas peaksin enne reisimist võtma probiootikume?
Kas peaksin võtma antibiootikume profülaktiliselt?
Kas loperamiidi on ohutu kasutada?
Kui kaua see peaks kestma?
