Diarrhoea teithwyr: sut i'w hatal

Mae prophylaxis yn gweithio'n llai effeithiol na'r cynhyrchion sy'n awgrymu, ac mae gwybod beth i'w wneud pan ddigwydd yn bwysicach na'i atal.

Beth mae prophylaxis yn ei gyflawni

Mae dadansoddiadau meta o prophylaxis probiotig [1][3][4] a phrobiotigau o gymharu â rifaximin [2] yn dod o hyd i leihau cymedrol yn y digwyddiadau. Mae 'cymedrol' yn y gair gweithredol: dyma leihau risg cymharol mewn cyflwr cyffredin, nid diogelu.

Nid yw prophylaxis gwrthfacterol yn cael ei argymell yn gyffredinol ar gyfer teithwyr cyffredin. Mae'n gyrru gwrthryfel, yn cynnwys effeithiau ochr, ac yn hyrwyddo colonisation gyda phathogenau gwrthfacterol y mae teithwyr wedyn yn eu dod adref — cost ar lefel poblogaeth a dalwyd am gyfleustra ar lefel unigol.

Prophylaxis ar gyfer diarrhoea teithwyr

Mesurau diogelwch bwyd a dŵr

"Berwi, coginio, plicio neu anghofio" yw cofiedig ac yn rhannol effeithiol — mae astudiaethau o ddilyniant yn dod o hyd i deithwyr sy'n dilyn y rheolau'n agos yn dal i ddod yn sâl ar gyfraddau sylweddol, yn bennaf oherwydd bod yr ymddangosiad yn digwydd mewn ceginau na allant eu gweld.

Mae'r mesurau sydd â'r dychweliad gorau: dŵr potel neu dŵr wedi'i drin gan gynnwys ar gyfer dannedd, osgoi iâ o darddiad anhysbys, bwyd poeth a gynhelir yn boeth yn hytrach na'i gadw'n gynnes, a ffrwythau rydych chi'n plicio eich hun. Mae hylendid dwylo cyn bwyta yn gwneud mwy na'r rhan fwyaf o osgoi bwyd.

Pan nad yw'n dim ond diarrhoea teithwyr

  • Gwaed yn y stool neu ffliw uchel. Ceisiwch ofal meddygol yn hytrach na thrin eich hun.
  • Symptomau sy'n para dros wythnos. Ystyriwch achosion parasitig, sydd angen triniaeth wahanol.
  • Dehydradiad difrifol, yn enwedig mewn plant a theithwyr hŷn.
  • Symptomau'n dechrau ar ôl i chi ddychwelyd. Dywedwch wrth y clinigwr ble rydych chi wedi bod; mae'n newid y gwahaniaeth.
  • Symptomau parhaus ar ôl epizod aciwt. Mae IBS ôl-heintus yn ganlyniad a gydnabyddedig ac sydd dan ddiagnosis isel.

Mae'r pwynt olaf hwn yn haeddu sylw: mae lleiafrif ystyrlon yn datblygu symptomau coluddyn parhaol ar ôl epizod heintus. Os nad yw eich coluddyn erioed wedi dychwelyd i'r normal ar ôl taith, mae hynny'n ffenomen a gydnabyddedig ac nid rhywbeth a ddyfeisiwyd gennych — gweler ein safle tystiolaeth coluddyn-yng-nghymdeithas.

Cwestiynau cyffredin

Ddylwn i gymryd probiotigau cyn teithio?
Mae meta-ddadansoddiadau'n canfod gostyngiadau cymedrol [1][2]. Risg isel, cost isel, disgwyliadau isel.
Ddylwn i gymryd gwrthfiotigau'n rhagataliol?
Yn gyffredinol, na. Mae ymwrthedd a sgil-effeithiau'n drech na'r budd ar gyfer teithio cyffredin.
Ydy loperamide yn ddiogel i'w ddefnyddio?
Ar gyfer rheoli symptomau mewn salwch anddifrifol, ydy. Osgowch ef gyda gwres neu waed yn y carthion.
Am ba hyd ddylai bara?
Mae'r rhan fwyaf o achosion yn clirio o fewn ychydig ddyddiau. Y tu hwnt i wythnos, ewch i gael eich asesu.
