Diarreia dos viajantes: o que a previne
A profilaxia funciona menos bem do que os produtos sugerem, e saber o que fazer quando acontece é mais importante do que preveni-la.
O que a profilaxia alcança
Meta-análises da profilaxia com probióticos [1][3][4] e de probióticos comparados com rifaximina [2] encontram reduções modestas na incidência. Modesto é a palavra-chave: estas são reduções de risco relativo em uma condição comum, não proteção.
A profilaxia com antibióticos geralmente não é recomendada para viajantes comuns. Ela promove resistência, traz efeitos colaterais e favorece a colonização por organismos resistentes que os viajantes levam para casa — um custo a nível populacional pago por uma conveniência a nível individual.

Precauções com alimentos e água
"Ferva, cozinhe, descasque ou esqueça" é memorável e apenas parcialmente eficaz — estudos de adesão mostram que viajantes que seguem as regras rigorosamente ainda ficam doentes em taxas apreciáveis, em grande parte porque a exposição acontece em cozinhas que não podem observar.
As medidas com o melhor retorno: água engarrafada ou tratada, inclusive para escovar os dentes, evitar gelo de origem desconhecida, comida quente servida quente em vez de mantida morna, e frutas que você descasca. A higiene das mãos antes de comer é mais eficaz do que a maioria das evicções alimentares.
Quando não é apenas diarreia dos viajantes
- Sangue nas fezes ou febre alta. Procure atendimento médico em vez de se auto-tratar.
- Sintomas que duram mais de uma semana. Considere causas parasitárias, que necessitam de tratamento diferente.
- Desidratação severa, particularmente em crianças e viajantes mais velhos.
- Sintomas que começam após o seu retorno. Informe ao clínico onde você esteve; isso altera o diagnóstico diferencial.
- Sintomas persistentes após um episódio agudo. A SII pós-infecciosa é um resultado reconhecido e subdiagnosticado.
Esse último ponto merece atenção: uma minoria significativa desenvolve sintomas intestinais duradouros após um episódio infeccioso.
