Diarréia dos viaxeiros: que a prevén
A profilaxe funciona menos ben do que os produtos implican, e saber que facer cando sucede é máis importante que previrlo.
Que logra a profilaxe
As meta-análises da profilaxe con probióticos [1][3][4] e dos probióticos en comparación coa rifaximina [2] atopan reducións modestas na incidencia. Modesto é a palabra clave: estas son reducións do risco relativo nunha condición común, non protección.
A profilaxe antibiótica xeralmente non se recomenda para viaxeiros ordinarios. Impulsa a resistencia, ten efectos secundarios e promove a colonización con organismos resistentes que os viaxeiros logo levan a casa — un custo a nivel poboacional pagado por unha conveniencia a nivel individual.
Profilaxe para a diarréia dos viaxeiros
Precaucións con alimentos e auga
"Fervelo, cociñalo, pelalo ou esquécelo" é memorable e só parcialmente efectivo — os estudos de adherencia atopan que os viaxeiros que seguen as regras de cerca aínda enferman a taxas apreciables, en gran parte porque a exposición ocorre en cociñas que non poden observar.
As medidas cun mellor retorno: auga embotellada ou tratada, incluíndo para os dentes, evitar xeo de procedencia descoñecida, comida quente servida quente en vez de mantida morna, e froita que peles ti mesmo. A hixiene das mans antes de comer fai máis que a maioría das evitacións alimentarias.
Cando non é só diarréia dos viaxeiros
- Sangue nas feces ou febre alta. Busca atención médica en vez de auto-tratarte.
- Síntomas que duran máis dunha semana. Considera causas parasitarias, que necesitan un tratamento diferente.
- Deshidratación severa, particularmente en nenos e viaxeiros maiores.
- Síntomas que comezan despois de regresar. Dille ao clínico onde estiveches; cambia o diagnóstico diferencial.
- Síntomas persistentes despois dun episodio agudo. O SII post-infeccioso é un resultado recoñecido e subdiagnosticado.
Ese último punto merece atención: unha minoría significativa desenvolve síntomas intestinais duradeiros despois dun episodio infeccioso. Se o teu intestino nunca volveu ao normal despois dun viaxe, ese é un fenómeno recoñecido e non algo que inventaches — consulta o noso sitio de evidencia intestino-cerebro.
